Insurance Plans We Work With

The plans listed below are commonly used for outpatient behavioral health and substance use treatment services. Acceptance and coverage are subject to network participation, benefit design, provider credentialing, medical necessity, and authorization requirements.

Government & Public Programs

American Indian Health Program (AIHP / AHCCCS American Indian Program)
Services may be available to eligible American Indian and Alaska Native members. Coverage is subject to AHCCCS eligibility rules and tribal program requirements.
AHCCCS (Arizona Medicaid)
Coverage is plan-specific and requires active enrollment. Prior authorization may be required for certain services and levels of care.
Medicare
We accept Original Medicare and select Medicare Advantage plans. Coverage depends on benefits, medical necessity, and plan rules.

Good-Faith Estimates

Patients who are uninsured or choose not to use insurance may request a good-faith estimate of expected charges in accordance with the No Surprises Act. Estimates are available upon request prior to services.

Insurance coverage is not a guarantee of payment. Final patient responsibility is determined by the insurance carrier based on eligibility, benefits, medical necessity, authorization requirements, and claim adjudication.
